As a commitment to our relationship, it is part of our process to always provide a copy of the patient’s discharge instructions and/or a doctor’s letter so that your patient’s care is seamless.
It is important to note that we are always willing to discuss what is best suited to you and your client, without any obligation on your part, prior to any referrals you may or may not choose to make, and if we are aware of any potential conflicts of interest to you and your clients, we will be sure to disclose and discuss these with you.

If you are referring a patient to one of our hospitals, please download and complete our pet referral form (see below) and return it to our office by mail or fax, along with any records pertaining to the patient’s past and present conditions. Please include any pertinent or recent lab work as well.
